ELECTROCARDIOGRAPHIC OBSERVATIONS OF ENTERIC FEVER

Abstract
One hundred and twenty-four electrocardiograms obtained from 95 cases of enteric fever were analyzed and the results were compared according to the duration of illness, degree of fever and hemobglobin level. In addition, in some of the cases with abnormal electrocardiograms on admission, a follow-up observation was made.
Ninety abnormal findings were noted in 79 out of 95 cases, the incidence of patients with abnormal electrocardiograms being 83.2%. Among the major abnormalities. sinus tachycardia was noted in 28.4%, ST segment and T wave changes in 25.2%, prolongation of P-R interval in 17.9% prolongation of Q-T interval in 7.4% and left ventricular hypertrophy also in 7.4%.
Sinus tachycardia and ST segment and T wave change were more common in the third to fourth week of illness wherezs prolongation of P-R interval was more frequent after the fourth week.
Incidences of sinus tachycardia, prolongation of P-R interval, ST segment and T wave changes in thowe with hemoglobin level of 6.0 gm.% or less were common but prolongation of Q-T interval was less frequent in those with higher hemoglobin level.
No close correlation was noted betweenm the grade of fever and the incidence of prolongation of Q-T interval or ST gegment and T wave changes. However, in those with high fever, prolongation of P-R interval was less common, and sinus tachycardia was more frequemt as anticipated. Changes in St segment and T wave had disappeared within 10 days in most of the cases but prolongatio of P-R interval persisted more than 10 days in more than 50% of the cases.
Electrocardiographic changes in enteric fever were probably brought about by multiple factors, but it was felt that ST segment and T wave change, and prolongation of P-R interval were frequently secondary to myocarditis.
